I hereby agree to and authorize the creative use of my name, title, photograph, and voice in any positive campaign announcement, advertisement, publication, literature, newspaper, radio, television, public announcements, other forms of media outreach, and fundraising activities in both digital and hard copy in support of Shaun Mena for state representative of district 23.
